Performance Physical Therapy will verify your insurance benefits during the first visit. Colorado allows “direct access” to physical therapists. This means you can see a physical therapist without a medical referral. However, some insurance plans require a medical referral for optimum reimbursement. The links below will help you find the member contact information for your carrier if you would like to review your plan benefits and referral requirements. Web links and phone numbers for insurance plans can also be found on the back of most members’ cards.

Co-pays, co-insurance, and deductibles are due at the time of each visit.

We are also able to accept out-of-network benefits for other insurance plans.
